How to Apply for a Duplicate Voter ID Card? [Offline]

If you lose your Voter ID Card or if it gets destroyed due to any unexpected incidents, you need to apply for a duplicate Voter ID Card, a copy of the original Voter ID Card. Here is the step-by-step procedure to get a duplicate voter ID card if you lose your original Voter Card:

Initially, lodge a complaint (Fire an FIR) at a nearest police station informing them about the loss of your Voter ID Card in order to avoid the misuse of the card by others.

Preserve the hard copy of the FIR safely, so that it can be used for further reference, if required.

Now, get the Form 002 from any regional election commission office or you can simply download it from the official Election Commission website of your state. This form is actually for those who are applying for a duplicate voter ID card.

Fill in all the details in the Form 002 and verify it twice whether the details mentioned in the form are correct or not.

After filling the form, attach a photocopy of the FIR along with the form 002 to get the lost Voter ID Card.

Along with the FIR, you need to attach some other documents as a proof of identity. Here is the list of documents that are required to attach with the FIR:

Birth Certificate (DOB)
Photo Identity Proof
Address Proof such as Adhaar card, Ration card, PAN Card, Bank passbook, Passport or a driving license.
Residence proof such as current bill, telephone bill or gas bill.

Place all the documents together in an envelope and submit them to any of your nearest Chief Election Commission office in your city.
